Prequalification | Documents

Please have the following documents ready for electronic upload with submission. It is suggested that all documents are gathered prior to beginning the prequalification process. The name and description fields of each document must be entered for file to upload successfully. All financial information associated with the prequalification process is kept strictly confidential.

Following is the list of required documents:

  • Completed & Signed W-9 Form

  • Certifications
    – WBE, MBE, DBE, etc.
      
  • Certificate of Insurance
    – Including endorsements
    McGough Insurance Requirements
    Sample Certificate of Insurance
      
  • OSHA 300 Form & 300A Form
    – Provide three year history
    – If you are exempt from OSHA record-keeping, provide a statement with reason for your exemption
      
  • EMR Letter of Verification
    – Provide three year history

  • Company Safety Manual
    – Compressed PDF format
    – If PDF file is >20 MB, please contact McGough Prequalification Department at subprequal@mcgough.com
      
  • Letter of Bondability/Bonding Capacity
    – DO NOT SUBMIT BOND
      
  • CPA Prepared Financial Statement
    – All financial information associated with the prequalification process is kept strictly confidential
      
  • Quality Assurance/Quality Control Manual
    – If your company has a QA/QC Program
